Tinkertoy12020-10-20The state seriously needs to check this place out a little further. My Dad, with early dementia was admitted there after heart issues at the hospital for rehabilitation/therapy. We would call to check on him or talk to him and the phones would ring for 5 minutes without an answer. They are observing very strict COVID rules. Therefore, we could not truly tell what kind of treatment he was receiving. We felt helpless after phone conversations in which he would beg us to come get him. He said that he was cold and only provided a sheet, he was thirsty, hungry and handled roughly. He is quite frail. His med list was ignored, he has celiac disease and cannot have gluten. He was fed whatever they had and has paid for it. We finally got him transferred and he is much happier. Do not take your loved ones here!!!I am a friend or relative of a current/past resident

Rhonda8773802016-10-10I placed my mother into Grand Boulevard Health & Rehab Center today. She is in the nursing home part. I know very little about it, but the location is very convenient for me so I can go and check on her daily. That’s one of the main reasons I wanted that location. What they serve my mother at lunch looks really nice. It is good. Her room is semi-private, and it has a bed. It has a bath that they share, and you can bring your own television and some things from home. So far, everyone has been very nice. I just dropped my mother off there. Everyone seemed nice. It was the weekend staff. Everyone seemed very helpful. I really don’t know that much about the facility because I just got there, but it was clean, the food looked great, and everyone was nice to my mother -- which were the right features for me.I am a friend or relative of a current/past resident
lilredridinghood2015-08-05I was dropping a parent off and entered to find no staff to greet me- just 3 or 4 patients tied into wheelchairs flopped sideways, others siting in their chairs drooling, it was sad and depressing...to get to the Rehab part one must travel through the long term stay part of the facility. Eventually located a staff member who told me to walk the rest of the hall to find 'Mary Ann' and hand her the papers from the hospital. This place turned out to be so bad I refused to let my teenager walk through what I had walked through. We had NO choice but to leave our loved one at the time and believe me, after the way they treated our parent we got them out as fast as we could. They did NOT follow doctor's orders, set my expectations as to updates and never followed them, barely returned calls unless I called repeatedly, NEVER notified me when they had to send my parent to E.R.- how would you like to have to track your loved one down? STAY AS FAR AWAY FROM THIS FACILITY AS POSSIBLE. We filed formal complaints. As long as they get their money from the source, they do not care about the patient. JaniceM2019-07-17• Meals have been, in Janice’s experience, to a large degree, inedible because of being cold, lacking margarine or butter, and no offers of condiments which are generally always on the delivery carts. Asking for the meal to be reheated was sometimes successful, other times not. We understand that she has been on a renal diet, which frankly sucks, but is it not better to have the patient eat SOMETHING rather than refuse to eat at all? Not suggesting ignoring the prescribed diet, but occasionally they should be able to have a more liberalized choice. This was the case when she was at UAB and Lakeshore, but both of those facilities seemed to have higher staffing levels, or a better patient to staff ratio and, generally speaking, were FAR more responsive to their patients. • In the case of taking the word of staff over patients, staff always wins. One case in point was the issue about Janice refusing to take a shower, an event that I witnessed that did not happen as discussed on June 5th. • Promises to schedule showers on specific days do not happen unless I or Janice pushed the issue. In just short of 7 weeks as a patient, Janice received 4 showers. • Some CNAs and nurses are exceptionally good; I can’t praise Gretchen and Reba enough. Gretchen and Reba are consistent and caring…. always (in our experience.) Donna is excellent in following up on patient needs and “fixing” problems. Always cheerful and positive. • Not being able to make a timely exit after 4 pm due to “door alarms” is particularly annoying. I understand the need to keep dementia patients from wandering out of the building, but it is no less annoying. Don’t have any suggestions for an alternative. • Finally, it will be interesting to see what the CT scan results are after having complained about Janice’s’ chest pains and coughing for the past 4 weeks with no definitive action taken until she is discharged. • One particular annoyance was why I had to, for nearly 4 weeks, drive from the north side of the bay to GB to take Janice for dialysis when another GB patient was being transported for her appointment (scheduled about 10 minutes prior to Janice’s). This resulted in an extra 58 mile round trip for me 3 days a week. The past two weeks, GB has been transporting Janice with the other GB patient to the same treatment site. • Gastrointestinologist's office finally called a week or so after discharge. Asked if Janice had had a colonoscopy in the past 5 years, Janice said she did abut three years agos. Janice would told that she would have to see that doctor, they would not schedule an appointment as requested by GB. I am/was a resident of this facility

We took care of her there until she passed away. The time mom spent at the nursing home was great. The staff was very caring ,and every one was very helpful in helping us get her back to Ga . our family is very greatful for such a wonderful staff. my moms name is Barbara Howren.Thank you to all that helped us and loved us on our heartbreaking days.Having such wonderful people was a blessing.Thank you all.The Barbara Howren family. An Evolving Grand Boulevard Initiative

Grand Boulevard Hook Up Sites -

For thirteen years, communities along El Camino Real (State Route 82) have participated in the Grand Boulevard Initiative (GBI) to revitalize the roadway into a vibrant, people-friendly place. GBI has fostered the development of ten Guiding Principles and has seen the endorsement of those principles by every community along the corridor. Numerous studies and plans have been designed to give cities, counties and other agencies a path forward to upgrade land uses, public services and infrastructure; and, well-managed higher density development. Since roadway and development projects are aligned with GBI’s vision and the guiding principles in the implementation process, GBI has reached a crossroads in which the group will evolve to refocus its efforts on mobility along the corridor. The purpose of this paper is to recognize the achievements of GBI and its partner agencies and to describe a new approach for GBI, designed to assure its continued effectiveness in a changing environment.

Moving Foward

Task Force

Moving forward, the Task Force will no longer meet quarterly. In lieu of regular meetings, there will be annual convening(s) on specific topics that are trending and/or of interest to the members. Corridor tours may also be held in lieu of regular meetings. These events will engage the GBI Task Force members in addition to new stakeholders that are interested in El Camino Real topics.

Working Committee

In order to continue to share best practices along the corridor and explore ways to improve the mobility and transportation options along the corridor, the GBI Working Committee will transition to a similarly-structured El Camino Real Mobility Working Committee with less frequent meetings. The effort will be led by SamTrans (and any other interested agencies) and comprised of similar membership to the current Working Committee including transportation planners and engineers from San Mateo County and Santa Clara County jurisdictions along the ECR corridor. The group will meet quarterly, rather than monthly, and all in-person meetings will offer webinar/remote options.
